What is Community Inclusion?

Community Inclusion (CI) is an individualized service that provides support to the people we serve to engage in community-based activities that support education, recreation, socialization, and most importantly, personal development.

Community Inclusion creates, builds, and strengthens organic relationships with others in the local community, with the goal of fostering greater independence and societal inclusion.

Eligibility

Community Inclusion is available to individuals in or seeking retirement, or those who have participated in at least nine (9) months of Employment Services. Participants may go back to Employment Services at any time.

Success

Our success is measured by yours. You’re an individual, and your pathway to community connection should be as unique as you are.

Participants have a dedicated team that will regularly check in with you/your participant throughout your services to ensure successful connections and to provide support should participants want to try something different.

What Can You Expect?

We meet with participants to build service plans and discover their passions. Participants choose their activities according to what you/they want to do, and we provide the empowerment to make it happen.

  • Local, community-based activities and engagement opportunities.
  • Discover a social group that includes and celebrates YOU!
