ACTIVE ENGAGEMENT IN THE COMMUNITY

Community Inclusion provides coordination and support for people with disabilities to actively engage in their community. Our services focus on personal preference, bridge building, life enrichment, and use of integrated resources. A particular goal of Community Inclusion is helping people with disabilities build a natural support network to enable them to fully participate in activities along with others who have common interests.

Community Inclusion staff provide one-on-one assistance to each individual and their support team to develop a personalized plan that will result in connection and contribution to the individual’s community. Services are designed for individuals who are of retirement age or who have completed at least nine months of employment exploration.

> Relationships with a variety of individuals and the guidance to create a natural support network with less reliance on paid companions.

> Support to explore their preferences, interests, and potential.
